Students, you are expected to describe your planned enrichment activities (e.g., research, conference presentation) and provide confirmation of your travel dates, location(s), and mode(s) of transportation. You must also provide emergency contact information. You will receive a copy of your responses via email once you submit this form. If you are applying for support for your travel (e.g., Presidential Scholars Enrichment Award, OUR Conference Presentation Award), please wait for your award application to be approved for funding before completing this form.

The information you provide will allow Enrichment Programs to enter the relevant information in UConn's Concur travel system. Prior to completing this form, you must set up your profile in Concur and name Susan Ruggiero AND Tiffanie Itsou your delegates by following these instructions.
